Why Semen Analysis is Important
When diagnosing infertility, it is important to identify all possible contributing factors. Infertility can be caused by female factors, male factors, or a combination of the two. Semen analysis can help determine if the male is producing healthy, vital sperm. While routine sperm count tests at a medical reference laboratory can provide basic fertility information, a comprehensive semen analysis performed by a fertility lab can better assist couples trying to conceive.
What Factors are Measured?
Semen analysis is often the first male factor test initiated when addressing infertility. At Pacific Fertility Center, we examine:
- Sperm Count: We can measure the number of sperm present per milliliter in a single ejaculation. The normal count is anywhere between 40 million to 300 million sperm.
- Motility: To determine motility, we can examine the swimming ability of the sperm.
- Morphology: According to the World Health Organization, the quantity of normal-shaped sperm per milliliter should be approximately 30 percent.
- Velocity: To determine velocity, we can calculate the forward progression of the sperm. This identifies the quality of movement.
- Volume: We can measure the amount of semen in one ejaculation. Normal volume is approximately 2.5 milliliters. Low volume could indicate an issue with the prostate or it may suggest that the seminal vesicles are not producing enough fluid.
- Liquefaction: Healthy semen turns from a thick gel to a liquid within 20 minutes. Liquefaction is the time it takes for this to occur.
Additionally, the Kruger morphology test is an important aspect of semen analysis when diagnosing infertility. This method takes morphology a step further, specifically assessing the shape of the sperm head. In healthy males, at least 14 percent of the sperm have normal-shaped heads. Infertility may be indicated if that number is at or below four percent.
If there are no sperm present in an analysis, we can test the semen for seminal fructose, which is typically produced in healthy males. If fructose is not present, it could signify obstruction of the ejaculatory duct. Otherwise, it could indicate that the vas deferens or seminal vesicles are congenitally missing.
