Payment Policy

Many patients ask for information about when payment is due for services. The following information should address most circumstances; but if you have any questions, please do not hesitate to contact our Financial Administrator.

PRE-CYCLE LAB WORK

Before you begin any cycle of treatment, you may need certain tests that help determine the cause of your infertility. Payment is due in full at time of your service. Unfortunately, we cannot bill your insurance directly. We will provide you with a superbill so that you may discuss the possibility of coverage with your insurance carrier.

HOSPITAL SURGERY


If you need an in-hospital stay, we will attempt to pre-authorize all surgeries with your insurance carrier. A deposit is due at the time of the pre-operative visit. As a courtesy, we will bill your insurance for the physician’s fees. Any portion of the charges unpaid after your insurance benefits will be your responsibility. You should also expect to receive a bill from the hospital and the anesthesiologist.

FERTILITY MEDICATIONS

You may buy fertility medications at any pharmacy chosen by you and/or your insurance company. In some cases, you may need to purchase medications through us (for instance, on a Sunday when nearby pharmacies are closed). In such situations, we will make arrangements through our building pharmacy to be able to dispense your needed medications. Payment can then be made directly to the pharmacy on the next business day or through a credit card at time of purchase. This also applies to medication dispensed to a donor and/or surrogate. We do not accept returned medications.

IVF CYCLE FEES (ALL PFCLA PROGRAM PLANS)

Single cycle and Refund program fees are due in full one (1) week prior to the time you and/or your surrogate or donor start fertility medications. If applicable, we do require a letter from your insurance company stating that you have IVF coverage, and indicating the specific services they will cover. We will attempt to get pre-authorization for you. If your insurance does not cover 100% of the services you require, a deposit from you will be necessary before the cycle of treatment begins.

OUTSIDE LAB CHARGES/SHIPPING COSTS

Some of our lab tests are referred to an outside Laboratory for processing. This laboratory (such as MedScience or LabCorp) will bill your insurance for you and bill you for what insurance does not pay. Other laboratories, such as Reproductive Immunology Associate’s Laboratory in Van Nuys require that a payment accompany the specimen. While PFCLA does not make a charge for collecting these specimens, we do incur Federal Express expenses related to shipping. You are responsible for payment of packaging & shipping costs.

METHODS OF PAYMENT

For your convenience, we accept personal checks (U.S. dollars), cash and all major credit cards (VISA, MC, AMEX, Discovery).

REFUNDS

In the event of a credit balance, it is our policy to issue a refund promptly. This process could take up to 2 weeks from the time the refund is requested. All refunds are made by check. We cannot refund by cash or credit card.

CANCELLATIONS

If treatment is cancelled after it has been initiated for any reason, we will review your account and only charge you for procedures completed by the time of the cancellation. In some instances, such as when an IVF cycle has already been started, a cancellation fee is charged for non-medical cancellation excuses. Please check with our office for the amount charged.
